Join us for a talk by Barbara Katz, RN, MSN, member of New Haven Friends and author of the new book, Connecting Care for Patients, Interdisciplinary Care Transitions and Collaboration. This talk is free and open to health professionals and the general public. 

Barbara's talk will include:

  • why the American health care system often creates a disconnected, impersonal, and in some cases unsafe, experience for patients, especially those with serious illness.
  • examples of organizations and individuals who have found better ways to integrate the pieces of care for patients. 
  • Connected Care, a set of six strategies that health care professionals and organizations can use to connect the dots of care for patients. 
  • specific tools and techniques for patients and family members of patients who want to take more control of their health care and get better results. T

Barbara Katz, RN, MSN is Vice President of Clinical Program Development for VNA Community Healthcare in Guilford. She has worked in health care for over 40 years as a nurse, APRN, health educator, health care manager, trainer and consultant.
